Parishioner Request for Letter of Eligibility

A Letter of Eligibility is necessary for each Catholic Godparent or Confirmation Sponsor. This letter is to be obtained from the parish of which the godparent or sponsor is registered and presently worshipping. It must be the original letter stamped with the parish seal and signed by the pastor. This letter indicates that the godparent or sponsor meets all of the requirements, including attending Mass regularly, received all of the Sacraments of Initiation (Baptism, Eucharist, and Confirmation) and if married, married in the Catholic Church.

Please complete the following form to submit your request to Saint Agnes Parish.

**Please note that the form below only applies to registered parishioners of Saint Agnes Parish.  If you are a godparent or sponsor seeking a Letter of Eligibility and are registered at a different parish, please contact their parish office for the necessary paperwork.**

Acknowledgement

I understand and acknowledge that if I am deemed eligible and granted a Letter of Eligibility from Saint Agnes Parish, I MUST pick up the letter following one of the Sunday Masses (7am, 9am, 10:30am, 12pm (Mass in Spanish), or 5:15pm) or the Saturday Vigil Mass (5:15pm) from a Parish Priest.

I also understand and acknowledge that only a Parish Priest may give me this letter outside of the Main Church as I exit one of the above Masses and my letter CANNOT be given to me by a Parish Staff Member or be picked up at the Rectory Office.
